Targeted Case Management

Radiant Recovery and Counseling (RRCs) provides Case Management services to Children through Adults.  RRCS, does provide Case Management services to the various waivers:  DD, CADI, BI, EW and Radiant Recovery and Counseling does provide Targeted Case Management Services.  Case Management services at Radiant Recovery and Counseling primary goal is to connect individuals with all of the services they need.  The Case Management team at Radiant Recovery and Counseling will ensure the provision of quality support services that are delivered in the most efficient way.  Radiant Recovery and Counseling is devoted to the care of individuals we serve on a continuous daily basis.

Qualifications for Services

To qualify for TCM, individuals must complete a Comprehensive Evaluation to determine eligibility, be a recipient of a Prepaid Medical Assistance Program (PMAP) and meet the requirements for Minnesota Rule 79 case management services.

Services are available to individuals with Medical Assistance (MA) or utilizing CMMHC’s Sliding Fee Scale Discount Program.

Children’s Targeted Case Management (CTCM)


Our Children’s Targeted Case Management services are designed for children and youth (under age 18 or up to 21 in extended foster care) with serious emotional disturbances (SED). Our team works closely with families, caregivers, schools, and community providers to coordinate a comprehensive plan of care that meets each child’s unique social, emotional, developmental, and cultural needs.

What We Provide:

  • Comprehensive assessments and individualized service planning.

  • Coordination of mental health, educational, and social services.

  • Advocacy in school settings, juvenile justice, and healthcare systems.

  • Support in transitioning between levels of care (hospital, residential, outpatient).

  • Culturally and linguistically appropriate services.

  • Regular progress monitoring and family engagement.


Who We Serve:
Children and adolescents diagnosed with Serious Emotional Disturbance (SED) who require coordinated services across multiple systems, including those at risk of out-of-home placement or school disruptions.

Adult Mental Health Targeted Case Management (AMH-TCM)

Our Adult TCM program is built for adults (18+) living with Serious and Persistent Mental Illness (SPMI), helping them navigate complex life challenges through individualized care coordination, advocacy, and access to vital supports. We serve as a consistent, reliable partner in their recovery journey—supporting long-term stability and community integration.

Core Services Include:

  • Assessment and service plan development with the individual.

  • Connecting clients to housing, employment, medical care, therapy, substance use treatment, and legal services.

  • Crisis planning and prevention.

  • Advocacy with county, state, and community-based systems.

  • Routine follow-ups to adjust goals and address barriers.


Eligibility:
Adults with a qualifying diagnosis of SPMI, who require assistance to access and coordinate the mental health, medical, social, educational, and housing services necessary for improved functioning and recovery.

  • For most people your insurance should cover online therapy, however, we encourage you contact your provider to verify that it is covered. We accept all insurances including medical assistance and other state health plans. A reduced rate for uninsured private pay patients is available.

  • We offer services through out the week, as well as evening hours Monday through Thursday. Normally, a first appointment will be scheduled within 48 hours.

  • No, you can use your mobile phone or any other device. You will receive a link to download the app to your mobile device.

  • No, none of your sessions will be recorded. Your privacy is very important to us.

  • For our virtual or online therapy, we use a HIPAA compliant platform called TherapyIQ that you receive once you become our client. You automatically open in your web browser to start the session. This link will work on your computer or smartphone, or you can download the app to your smartphone. This will allow us to have secure therapy sessions virtually. Your therapist will assist you with a link they send you via email to the secure platform at the time of your appointment and will be there to walk you through any issues you may have.
